Information regarding 1095-Cs
How to request a 1095-C?

• You many request it in writing on a communication form in the group home backpack.
• You may call Regina at the ALSD office (281-888-9248) and request it verbally.
• You may email Regina at admin@alsdonline.net and request it in writing. 

What is a 1095-C?

• This form is required by the Affordable Care Act (ACA) and outlines your health insurance coverage details, including the months you were covered and the type of plan you had. 

What to do with your 1095-C:

• Keep it for your tax records: You will need this form when filing your individual income tax return. 
• Review the information carefully: Ensure all details regarding your coverage are accurate. 

Delivery method:

• If you request a copy of this form, your 1095-C will be mailed to your address on file or can be picked up in person at the office. 

Important Dates:

• If requested, you should receive your 1095-C by March 3, 2026.
