How to request a 1095-C?
- You many request it in writing on a communication form in the group home backpack.
- You may call Regina at the ALSD office (281-888-9248) and request it verbally.
- You may email Regina at admin@alsdonline.net and request it in writing.
- This form is required by the Affordable Care Act (ACA) and outlines your health insurance coverage details, including the months you were covered and the type of plan you had.
- Keep it for your tax records: You will need this form when filing your individual income tax return.
- Review the information carefully: Ensure all details regarding your coverage are accurate.
- If you request a copy of this form, your 1095-C will be mailed to your address on file or can be picked up in person at the office.
- If requested, you should receive your 1095-C by March 3, 2026.
